Prospective Puppy Owner Questionnaire

Hello.  We really appreciate you taking time to fill out this questionnaire, which is designed to help us get to know you.  This will also help us discover if we have the type of puppy which would fulfill your needs.  Please answer ALL of the questions & fill in the blanks, then click on the "Submit" button & it will be E-mailed to us.  Please only press the "Submit" button ONE TIME, even if you get an error message.  I do receive them as many times as you press the "Submit"  button.  Because this is NOT a secured site, & you might not feel comfortable giving out information online,  please feel free to print the questionnaire, fill it out & mail it to Von Himmel Rottweilers,  P.O. 4.  How did you find us & why are you interested in one of our puppies?    
 

5. Why do you want to own a Rottweiler?  (Pet, show, etc), and will this be your first time owning a Rottweiler?
 
 

6. If you have owned a Rottweiler in the past, where or from whom did you get your last Rottweiler?  (Classified ad, friend, reputable breeder, pet store, rescue, etc).

 

 
7. What happened to your last Rottweiler & how old was he/ she when it passed away?
  

8.  What is your occupation, how long have you had this job, & do you work at home?
 

9.  If you have had you present job less than 5 yrs, how long did you have your previous job & what did you do?
 

10.  Does  your job require you to travel?        


11.  If your job requires you to travel, how many times a month & for an average of how long and are you out of town, out of State,  or out of the country?
 

12.  When you are out of town, who takes care of your pets & where are they? (Boarded, stay at home w/dog sitter, relative or friend, etc.

13.  Do you plan to change jobs or move in the near future?  


14.  If yes, where will you move to (State, country), will you rent an apartment or house, or purchase a home,  what will your new job be, & can/will your pets be moving with you or be otherwise relocated?  If the pets will be relocated, to where, with whom, & for how long?

15.  Is your mate/spouse employed? If so, for how long & what does he/she do?  Is it a full or part time job?  In or out of the home?
 

16.  If you have other pets at home, how many, what are they (dogs &their breed, cats, hamsters, etc) & their ages & who primarily cares for them?   

 

17.  Are they spayed/neutered?  If not, why?                    

18.  Are you a Breeder?  If you are, do you title your dogs, how long have you been breeding,  what breed(s) do you have, and how many litters of each breed do you have each year?  

19.  Where would your puppy sleep during the day and at night?
  
 

20.  Where would your puppy be when you go on vacation?

21.  What do you feed your dog? Please list brand names & if it is kibble, canned, cooked, raw, etc.  and why you have chosen to feed this diet?  Is your dog doing well on this diet? (no itchy skin, stinky ears, tartar on teeth, hot spots, goopy eyes, etc)?
 

22.  Do you have a secure yard with adequate fencing for your puppy to play in?  If so, what material is it made of, how high is it, & is it in need of any repairs? 

23.  If not, what are you planning to do to have a safe & secure yard for your puppy?     

 
24. Would you object to an UNANNOUNCED IN HOME VISIT from either myself, my spouse, or a friend of mine whom announced to you they were at your home on behalf of Von Himmel Rottweilers?   If not, why?                           


25. Do you have children?   If you have children, how many, their ages,  & if they are adults,  do they live with you in your home?  
  

26.  If you are planning to have children,  when will you begin your family, how many children to you plan to have & how far apart?
 


 

27.  Who is/are your Veterinarian (s) & why have you chosen him/her/them?
 

28.  Do you believe it is necessary to vaccinate your puppy &/or adult dogs?  If you do, which shots do you give (Parvo, Distemper, Corona, Lyme, Bortadella, Lepto, etc) & how often? 
 

29.  How often does your vet suggest you vaccinate?  ( Bi-yearly, yearly,  every 3 yrs., no boosters, do a titer instead of vaccinating) 
 

30. Would you consider a different vaccination protocol suggested by a well known, research veterinarian who specialized in immunology or one of a Holistic Veterinarian?  If not, why?
 

31. Have you heard of world renowned research immunologist, veterinarian Dr. W. Jean Dodds, DVM, and her 2005 revised vaccination protocol?  If so, where?       


32.  Why do you think so many dogs of all breeds have so many health problems (i.e.: chronic disease,  hot spots, fleas, tartar, bad breath, poor coat, etc)?
  
 

33.  How do you plan to properly socialize your puppy?  

34.  Are you willing to enroll your puppy to at least TWO 8 week obedience classes by the time your puppy is  18 months old?     If not, why? 

35.  Are you willing to sign and honor a written contract with the breeder?   
 

36.  Would you be willing to share pictures of your puppy and allow visits from the breeder & stud owner throughout the life of the dog?    If not why? 
 
37.  When the puppy is two years old, will you take the dog in for OFA certification of it's hips & elbows, heart (by a board certified cardiologist), eyes  (by a board certified ophthalmologist) & have a baseline complete thyroid & blood work panel done ?  If not, why? 

38. Will you be willing to send a copy of each health test result and certificate to the breeder AND stud owner?   If not, why?   

39.  Are you willing to continue feeding the raw diet that the breeder recommends?  If not, why?  

40.  Should you lose your home and/or homeowners insurance, what would happen to your Rottweiler? 

41.  If you became divorced /separated from your mate, what would happen to your  Rottweiler and any other pets you have?
 
 

42.  What would happen to your dog in case of your terminal illness or death? 

Please understand that this is only a questionnaire to assist Von Himmel Rottweilers get to know you better, which will help us determine if we have the type of puppy which would best fulfill your needs.
